As a rule of thumb, at least two to three centimetres across — closer to four if the code holds a lot of data. The more it carries, the finer its grid, and the larger it must be printed.
Yes. Twelve digits are enough — we calculate the check digit. With thirteen, we verify it and reject a wrong one. One tip from practice: never print retail barcodes in red. Checkout scanners use red light, which reflects off red ink just as it does off white paper — the scanner sees a blank surface. The tool warns you about this.
